2024-25 Student Athlete Participation Paperwork

To All Student-Athletes,

Training camps are invitation only. Invited student-athletes will receive an email to their Fitchburg State student email address from the Director of Athletics, Matt Burke (mburke43@fitchburgstate.edu). The email will contain additional camp information.

We look forward to welcoming back our fall student-athletes to preseason training camps on the following dates:

Women's Soccer	Friday, August 16th	9:00 AM	Recreation Center
Men's Soccer	Friday, August 16th	9:45 AM	Recreation Center
Field Hockey	Friday, August 16th	10:30 AM	Recreation Center
Volleyball	Friday, August 16th	11:15 AM	Recreation Center
Men's & Women's Cross Country	Monday, August 19th	9:00 AM	Recreation Center

All student-athletes are still required to complete all department and medical paperwork. Completed sports medicine & student-athlete data forms are due Thursday, July 18th. Please fill out all forms completely and follow the instructions for submitting them.

It is MANDATORY that you complete the following forms:

Student-athletes are also required to send their immunizations to immunizations@fitchburgstate. edu

Information for other immunizations can be found by clicking HERE.

Those who do not complete their forms by the deadline (Thursday, July 18th), may lose their eligibility to participate.
