2023-24 Pre-Participation Medical Requirements

Prior to try-outs/participation on a Fitchburg State University athletic team, student-athletes must complete all medical requirements as outlined on our Sports Ware website.

If you are under the age of 18, please contact us directly as you will not be able to complete the medical requirements without a parent/guardian.

ALL STUDENT-ATHLETES

1. Got to www.swol123.net 
   
2. Enter your FSU student email address (i.e. jdoe123@student.fitchburgstate.edu) into the email box.
3. Enter your previous password or create a password by clicking “Reset Password”.  This will send an email with a link to create your new password to your student email account.  Do not hit “Join SportsWare”.  There is no School ID.
   
4. Read and electronically sign all outstanding “Forms”.  Instructions for this process may be found under “Notices and Handbooks”.
   
5. Complete any missing information under “Status” where marked with a red asterisk or marked "Required".  
   
6. Scan or photograph the front and back of your health insurance card and upload it where indicated in SportsWare.  All student athletes must have health insurance to participate and should have an insurance card in their own name.
   
7. Review all “Notices and Handbooks”.
8. Complete all questions in “History” with “Yes” or “No” providing details to the question to which you answer “Yes”.
9. If you are taking a banned medication (most commonly ADHD stimulant medications), submit the banned substance waiver information to your physician to complete and return back to our staff.  The waiver may be found on the SportsWare Dashboard.
10. Submit/update all vaccination records to Student Affairs.  https://www.fitchburgstate.edu/student-support/health-and-wellness/student-access-health-care/health-requirements

NEW/TRANSFER STUDENT-ATHLETES

1. Complete a baseline concussion exam found at http://www. concussionvitalsigns.com/. The instructions for completing this exam may be found on your SportsWare Dashboard.  Those who are participating in cross country or track (with the exception of pole vaulters) do not need to complete this exam.  If you completed this exam last year, you do not need to do it again.
2. Submit your Sickle Cell Trait Results utilizing the forms and instructions on your SportsWare Dashboard
3. New/transfer student-athletes will be scheduled for a required pre-participation screening by our team physician once the above steps are complete.  Student-athletes who are reporting for fall pre-season will be seen on the first day of their respective camp and do not need to make an appointment.
